Transferring your patient
records from another dentist to us is fast and easy.
Here are a few things to keep in mind:
Your existing records are not necessary
at the time of your first appointment with us.
At the time of your first appointment,
you can request that your records—written
and radiographs—be transferred from your previous
dentist by simply completing a written request in
our office. There is no need to call your previous
dentist prior to your first meeting.
At your first meeting with us, we will
review what records, including health histories
and radiographs, may need to be updated.
A fee may be applied to your account
from your previous dentist for transferring your
records. In this event, our office will credit your
account for the amount charged.
